Youth Encounter Internal Staff Forms
A collection of internal Youth Encounter forms for our staff to use throughout the client journey to support participant registration, referrals, programme delivery, outcome measurement, and organisational reporting. Please select the appropriate form below based on the participant's stage of engagement.
Use for: All new participants entering Youth Encounter Therapy, Mentoring, or Young Leaders programmes. This form must be completed by a parent, caregiver, or whānau before a young person begins participating. (Camps have separate registration forms for each camp, and event registrations are completed on arrival.)
Use for: Referring an existing Youth Encounter participant to another Youth Encounter service. Staff to complete this form when a current participant would benefit from additional support, another programme, or a different service pathway.
Use for: Participants who have just completed a session, activity, or programme. Collects immediate feedback about their experience, what they enjoyed, what they learned, and suggestions for improvement.
Use for: Referrers, parents, caregivers, or whānau to provide feedback after a young person has completed a programme. Captures observed changes, outcomes, and the impact of Youth Encounter's support.
Use for: Participants approximately 6+ months after completing a programme. Measures how they are progressing, how they have applied their learning, and the ongoing impact of their Youth Encounter experience.
Use for: Referrers, parents, caregivers, teachers, mentors, or other key support people approximately 6+ months after programme completion. Measures whether positive changes have been sustained and identifies any ongoing support needs.
Use for: Participants 3+ years after completing a Youth Encounter programme. Measures the long-term impact of their experience on areas such as identity, purpose, education, employment, relationships, resilience, and community contribution.
Use for: Community members, schools, partner organisations, and stakeholders. Measures Youth Encounter's broader impact within the community, including awareness, partnerships, community benefit, and perceived outcomes.
